Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.
Yes. A physician referral is required by your health insurance plan for you to be seen by a licensed therapist. We also offer treatments, services and/or products that do NOT require a referral and are available on a self pay basis.
We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.
As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.
Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.

– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.
Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.
We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.
Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.
The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.

“LiveWell is not your grandma’s physical therapy. Even the appearance of this facility is different – it looks like an uncluttered, streamlined health spa. It is sparkling clean and well maintained – I actually wanted to take a picture of the attractive bathroom! The staff is superb. Luz is so knowledgeable about how the body works and was able to explain it to me in clear terms what has happened to my foot (tendonitis), and what I need to do to overcome this problem. She is friendly, funny and firm with me when I whine. The rest of the staff is also first rate – Julie, Brittany and Melinda; and Kem, the new guy is fitting right into the group. My hour of therapy flies by and it’s fun! I’m very comfortable with any one of the staff guiding me through my work out.
My tendonitis hasn’t gone away, but it is improved. Normally now the pain is about a “1”, and even if I’ve shopped for hours, it only goes up to maybe a “5”. My balance has greatly improved and I’ve gotten into a routing of riding the stationary bike and exercising that I hope to continue. I’ve told all my friends if they need therapy to come to LiveWell.”

LiveWell PT has found the key and unlocked the secret to positive rehabilitation therapy. Luz and her staff are nothing short of AWESOME!! The sessions are always challenging, but it never increases my pain level. Most of the time my pain actually decreases during therapy. My range of motion has dramatically increased. My body is becoming more flexible and definitely stronger with every session. Luz and her staff are very attentive, professional and accommodating; always observant and monitoring my exercises making corrections where needed. I know that my recovery will be a long road ahead at best. Progress is slow, but the staff at LiveWell PT has shown me that there is light at the end of the tunnel.

“On arriving back in town after my knee replacement surgery in Houston, we stopped at the PT place I had called earlier and thought everything was set up with. We took the RX from our Dr. in and asked for an appointment time (Monday). They told us it would be at the end of the week. We got the paperwork and my daughter started hunting for someone that knew the importance of physical therapy for me. She called LiveWell and explained the situation. Ms Luz told her to bring me in at noon the next day and she would work me in on her lunch time and then get me scheduled with my Dr’s request. She and her help are terrific! They truly care about their patients. Oh yea! it hurt, but she explained the exercises I needed to do in order to gain my full mobility. She and her staff are punctual and make everyone feel important. Her office and therapy rooms are very clean and the machines, tables and equipment are very sterile. “Even though I’m only 22 years old I had a significant amount of back pain. My spine is slightly curved, so it seemed like every motion amplified my pain. So, I decided that I better get a hold on this before it got worse.
While here, I was taught how to quickly relieve my back pain once it started, and how to quickly relieve my back pain once it started, and how to stretch and exercise parts of my body that would make my back stronger. Now when I have back pain, I know how to make it better and I don’t have to suffer through back pain anymore.

Lack of physical activity for several years due to work hours and lack of motivation – led to 3 falls this spring, fortunately no fractures. My balance and endurance were very poor. I knew if I did not make an effort to improve my physical skills – I would be a prime candidate for a fall fracture with a dim future, possible w/c bound with the need for others to assist me. Pretty gloomy future. There are so many adventures that I have enjoyed throughout my life to settle for disability. I was referred here.
